Grey Bruce OHT seeks community council members

The Grey Bruce Ontario Health Team is seeking up to four new members to join its community council, also known as the Grey Bruce Ontario Health Team patient, client, resident, family and caregiver advisory council.

The community council will play a role in shaping the future direction of the health system in the local region.

Council members bring perspective based on the diversity of the community through their age, gender, background, culture and their own health service experiences. This volunteer opportunity asks members to join in monthly council meetings, often held online. The chance to participate in specific health team projects or working groups is also available.

The Grey Bruce Ontario Health Team is a group of organizations, healthcare providers and community members committed to working together to improve patient outcomes by coordinating all aspects of health promotion, disease prevention and integrated care as a unified team. For patients, families and caregivers, this will mean easier access to services, more effective communications with providers and better health outcomes.

“We see the role of the community advisory council to advance patient engagement and patient-centered care in all services provided by the Grey Bruce OHT,” said Barbara Horne, Grey Bruce OHT community council co-chair in a news release.

Early work of the local health team will focus on seniors’ health and people living with mental health and addictions, but members of the community council do not need to have direct experience in those areas.

The Grey Bruce Ontario Health Team is part of the province of Ontario’s health system transformation, initiated in 2019 and now with 54 health teams in various stages of development.

At maturity, the local health team will be clinically and financially accountable for delivering a full and coordinated continuum of care for the entire attributed population within Grey Bruce. The Grey Bruce Ontario Health Team will apply a health equity, quality improvement, population health, and co-design approach to its priorities. There are currently twenty-seven Grey Bruce Ontario Health Team partners actively working together on system transformation, with an open process for any new partner organizations to join in on this collaborative work.
